Unlocking Trust and Stability: A Journey of Positive Behaviour Support with Adnan

The Positive Behaviour Support team at Accomplish work directly with services, and the people they support. They were recently asked to support with a young man whose placement was at risk.

The young man, Adnan*, was living in a supported living setting. With a long history of drug misuse and mental health needs, he had also been involved with the criminal justice system. He had shown aggression also, which often corresponded with his drug use.

One of the drugs used by Adnan was marijuana and when he used it, would become hungry and particularly aggressive. As a result of his challenges, Adnan also struggled to budget or plan shopping – living in a supported environment that was his responsibility – and so rarely had enough food to meet his appetite. Combined with excessive hunger brought on by drug use, Adnan’s lack of food meant he often sought to take it from others – with potentially dangerous results. He also refused help from the staff team around budgeting or how to conduct himself in a safe way.

As he lived in a shared supported living setting, Adnan would try to take other people’s food and when challenged about that would become aggressive. The issue was becoming unsustainable and his placement was at risk. He had assaulted staff, police were involved and there were legal proceedings pending.

The PBS team were asked to come in at that point. They found that there was fear of Adnan, which was limiting how people could engage with him. He also didn’t want any support and wanted to stay in his room. The PBS team worked to build individual rapport with him, finding things in common which helped him open up and trust more.

The main Issues for Adnan were around money – essentially he wasn’t using what he had wisely. He was frustrated. He also didn’t want what he saw as ‘charity’ so staff couldn’t simply ‘give’ him additional food or make him meals.

Through carefully building a rapport with Adnan on a one-to-one basis, and using careful language, the team managed to start to help Adnan see the staff around him as a source of support and he began communicating more with them.

Initially staff, when making their own meals, would say ‘Oh, I’ve got some extra here, would you like some?’ That way he didn’t see it as them ‘helping’, they were simply sharing. Then, as he wasn’t hungry, he wouldn’t go into other people’s cupboards and the aggression lessened and he became more receptive to help.

That gave the team a good basis to work alongside Adnan so he could understand, and better use, his budget. They offered to help with shopping so he could make choices that would last him longer, and he agreed.

Since then things have continued to get better. He sees staff as support not as a ‘what are you doing here, I don’t need help” way. He has been going to work and while it is agreed that another placement will still be best for Adnan, the urgency for a move is gone and when he does move, it will be a positive choice rather than a placement breakdown.
