About
Ty Maes Rhos provides specialist residential care and support for up to nine adults of mixed gender living with Mental Health needs, mild Learning Disabilities (LD), Autism Spectrum Disorder (ASD), or Personality Disorder (PD).
Situated in the heart of Swansea, within walking distance of the city centre, Ty Maes Rhos offers a safe, therapeutic, and recovery-focused environment where individuals are supported to build independence, confidence, and resilience.
Our Approach
At Ty Maes Rhos, we deliver care through a trauma-informed and person-centred approach, recognising the importance of safety, trust, empowerment, and choice. Our team works closely with each person to understand their experiences and aspirations, ensuring support is compassionate, consistent, and collaborative.
We are skilled in supporting individuals who may experience emotional distress, self-harm, suicidal ideation, or behaviours that challenge, promoting stability through proactive, recovery-based support and meaningful engagement.
Everything we do is guided by our EPIC Values:
Mental Health Relapse Prevention Plans
Each individual has a relapse prevention plan built on a traffic-light system:
Green: Everyday wellbeing and stability
Amber: Early signs or triggers
Red: Indicators of significant concern
Plans are developed collaboratively with the person, their family, and professionals involved in their care. This enables early recognition and intervention using strategies such as distraction, de-escalation, sensory regulation, or prescribed medication.
Recovery and Outcomes
We use the Recovery Star Model, an evidence-based tool recognised by the Department of Health, to measure progress in key areas such as:
Managing mental health
Building self-esteem and confidence
Developing daily living and vocational skills
Building positive social connections
