About
Oakhill is a residential service offering specialist rehabilitation support for individuals with an Acquired Brain Injury (ABI). We understand that no two people—and no two brain injuries—are the same. That’s why our care pathways are personalised, flexible, and built entirely around each individual's needs, strengths, and aspirations.
Oakhill supports up to seven adults in a comfortable, home-like environment designed to encourage independence, promote recovery, and support meaningful life outcomes.
Our Approach
Our core mission is to provide the highest quality of care, tailored to the specific needs and goals of every person we support. Through person-centred planning, we focus on life skills development, emotional wellbeing, and achieving real, sustainable progress.
We believe that a life filled with purpose, connection, and fulfilment is possible for everyone—regardless of their disability or challenges. We offer a warm, supportive environment that nurtures growth and fosters personal achievement.
Outcome-Focused Rehabilitation
At Oakhill, progress is not only encouraged—it’s measured. We use a range of clinically validated tools to track and celebrate individual development:
-
Goal Attainment Scaling (GAS Goals) – Tracks personalised goals, measuring success in meaningful areas of life.
-
BIIES (Brain Injury Independence and Emotional Scale) – Measures living skills and emotional wellbeing.
-
SASNOS (St Andrews Swansea Neuro-behavioural Outcomes Scale) – An internationally recognised tool for tracking engagement, emotional regulation, cognition, communication, and social behaviour.
These tools allow us to produce clear, graphical evidence of progress for assessments and reviews, ensuring each person’s support plan evolves in line with their journey. Progress and plans are reviewed monthly with the person supported and their Key Worker to celebrate achievements and make necessary adjustments.
