Isfryn

Our primary goal is to provide the highest quality care that promotes health, happiness, and independence. Isfryn offers a warm, homely environment where individual needs and aspirations are at the centre of all we do. Through person-centred plans, we support life skills development to help people achieve their goals and live as independently as possible. We firmly believe that a good life with positive outcomes is achievable for everyone, regardless of their disabilities.

We are dedicated to delivering measurable and meaningful progress for the people we support. To track this, we use the Goal Attainment Scale (GAS Goals), alongside clinical tools such as:

• BIIES (Brain Injury Independence and Emotional Scale) — measuring independent living skills and emotional well-being

• SASNOS (St Andrews Swansea Neuro-behavioural Outcomes Scale) — assessing social interactions, relationships, engagement, emotional control, cognition, inhibition, and communication

These tools help us present clear, graphical evidence of progress to guide assessments and reviews. Support plans are reviewed monthly with Key Workers and the individuals themselves, focusing on active goals, celebrating achievements, and adapting support where needed.

Isfryn has eight en-suite bedrooms, communal lounges, a kitchen, supported bathroom facilities, and a spacious garden. The service is ideal for people aiming to reintegrate into the community, with access to activities such as walking and fishing. Located on the peaceful side of the Swansea valley, Isfryn is close to local shops, pubs, restaurants, a leisure centre with gym and swimming pool, and a cinema.
