About

Hazeldene supports up to six people with Mental Health needs, Learning Disabilities, and Acquired Brain Injuries. With over 25 years’ experience in providing slow stream rehabilitation for individuals with Acquired Brain Injury and complex needs, our aim is to maximise independence in a comfortable, supportive environment. We help individuals achieve their personal goals and aspirations.

Our Support

We encourage and motivate each person to feel positive about themselves. Our support focuses on building confidence and enabling individuals to engage in activities that matter to them, ultimately helping them lead happy and fulfilled lives. Community integration is a strong focus at Hazeldene, with ongoing support to help people progress towards supported living.

Our Approach

Our primary goal is to deliver the highest quality care tailored to each individual’s needs. We provide positive, flexible, and encouraging support. Using a range of communication tools, we ensure each person is actively involved in shaping their own care plan.

Our dedicated and experienced staff go above and beyond to help people reach their full potential. We support individuals in understanding their Mental Health, Acquired Brain Injuries, and Learning Disabilities, developing coping strategies to manage anxieties. We also work on building independent living skills and social awareness, helping people to live successfully in the community.

Our Accommodation

Hazeldene is a spacious home featuring a well-equipped lounge, dining room, multi-sensory room for relaxation, and an art room for creative activities. The first floor has five bedrooms with a communal bathroom, while the ground floor includes one en-suite bedroom.

Located in Morriston, Swansea, Hazeldene offers easy access to the local shopping centre and amenities.
