Personalised support

We offer everyone we support a person-centred, strengths based assessment, that highlights their existing abilities and how specialist support can enhance their independent living skills. We support people to retain their full potential and to make their own choices, so they live the life they want to without limitations.

We can also provide therapeutic support for people following discharge from acute hospital admissions and who may require additional support to regain, maintain and develop their living skills within a community setting.
